What Is Telehealth?
Telehealth involves the delivery of healthcare services using electronic communications when the patient and healthcare provider are in different locations.
Telehealth may include secure video visits, electronic communications, review of medical information, remote patient education, and other technologies used to facilitate healthcare services.
Patient Consent
By participating in a telehealth visit with Sky’s the Limit Health & Wellness, you acknowledge that you have been informed about the nature of telehealth services, including their potential benefits, limitations, and alternatives.
You understand that you may ask questions about telehealth before or during your visit and may withdraw your consent to receive telehealth services, subject to applicable law and the limitations of care already provided.
Potential Benefits
Telehealth may provide benefits including:
Improved access to healthcare
Reduced travel
Greater convenience
Improved access to follow-up care
Continuity of care
Remote review of laboratory results
Medication-management follow-up
Access to certain healthcare services when an office visit is not necessary
Potential Risks and Limitations
Telehealth also has limitations and potential risks, including:
Technology or internet failure
Interrupted audio or video
Limitations of a remote physical examination
Delays in evaluation or treatment
Potential privacy or security risks associated with electronic communication
The possibility that an in-person examination or additional testing will be required
Incomplete information if relevant medical records are unavailable
Your provider may discontinue or reschedule a telehealth visit if the technology is inadequate to provide appropriate care.
Patient Location
At the beginning of a telehealth encounter, you may be asked to confirm your current physical location.
Telehealth services may only be provided when the treating healthcare professional is legally authorized to provide care to a patient in the jurisdiction where the patient is physically located.
Being an established patient of the practice does not guarantee that a telehealth visit can be conducted from every state or location.
Identity Verification
For patient safety and privacy, you may be asked to verify information such as your:
Full name
Date of birth
Current physical location
Telephone number
Emergency contact information
Photo identification when appropriate
Privacy and Confidentiality
Sky’s the Limit Health & Wellness will take reasonable steps to protect the privacy and confidentiality of telehealth encounters in accordance with applicable privacy and healthcare requirements.
Patients should participate from a private location whenever possible and take reasonable precautions to prevent others from hearing or viewing confidential medical discussions.
Recording Telehealth Visits
Telehealth visits will not be intentionally audio- or video-recorded by the practice without appropriate notice and authorization when required.
Patients should not record a telehealth encounter without first obtaining permission and complying with applicable law.
Medical Records
Information obtained during a telehealth encounter may become part of your medical record, including assessments, diagnoses, treatment recommendations, prescriptions, laboratory orders, referrals, and follow-up instructions.
Prescriptions
A telehealth appointment does not guarantee that medication will be prescribed.
Prescription decisions are based on your medical history, clinical evaluation, applicable federal and state requirements, potential contraindications, and the professional judgment of the treating provider.
An in-person examination or additional evaluation may be required before certain medications can be prescribed.
Laboratory Testing and Diagnostic Services
Your provider may determine that laboratory testing, imaging, EKG testing, an in-person physical examination, or specialist consultation is necessary.
Failure to complete recommended testing or follow-up may affect the ability to continue or safely provide certain treatments.
Emergencies
Telehealth is not a substitute for emergency medical care.
If you are experiencing a medical emergency, severe or rapidly worsening symptoms, or believe immediate medical attention is necessary:
CALL 911 OR GO TO THE NEAREST EMERGENCY DEPARTMENT.
Do not wait for a telehealth appointment, website message, email response, or routine callback.
Technical Problems
If a telehealth appointment is unexpectedly disconnected, the provider or practice may attempt to reconnect or contact you using the telephone number provided.
If the connection cannot be restored and your medical concern requires timely evaluation, you may be instructed to obtain in-person care.
Alternative Care
You understand that alternatives to telehealth may include an in-person office visit, urgent care, emergency care, laboratory or diagnostic testing, or evaluation by another healthcare professional.
Your provider may recommend an alternative method of care when telehealth is not clinically appropriate.
Patient Responsibilities
By participating in telehealth, you agree to:
Provide accurate and complete health information.
Provide an accurate current physical location.
Inform the provider of medications, allergies, medical conditions, and relevant health changes.
Participate from a reasonably private and safe location.
Follow recommended testing and follow-up instructions.
Seek emergency care when instructed or when an emergency occurs.
Ask questions when you do not understand your treatment plan.
Acknowledgment
By proceeding with a telehealth appointment, you acknowledge that you have had an opportunity to review information regarding telehealth, including its potential benefits, risks, alternatives, and limitations.
Where a separate written or electronic consent is required, Sky’s the Limit Health & Wellness may require completion of additional consent documentation before providing telehealth services.
